Send a deposit for Camp Stanley, Stanleywood or Combo Rate with this application. Deposits are refundable until December 1, 2011. All payments for Camp Stanley and/or Stanleywood must be received no later than May 15, 2012.
All checks must be made out to Camp Stanley for the Performing Arts, Inc.
Rules & Regulations: The camper and parent agree to abide by all of the rules and regulations established by Camp Stanley for the Performing Arts, Inc. including, without limitation, those relating to enrollment and withdrawal of campers.
Dismissal of Camper: Enrollment at camp may be terminated at any time for the health, safety or welfare of the camper and/or the camp. No refunds will be made.
Permission to Participate: Parent grants camper permission to participate in all camp activities and understands that accidents and injuries may occur in the natural course of participation in such activities.
Miscellaneous: Campers may appear in photographs, DVD or videos used for promotional purposes.
Once your payment is received, the Directors from Camp Stanley will contact you directly. This is the time they will provide you with all necessary documents in order for your child to participate. To be eligible for enrollment at Camp Stanley for the Performing Arts, Inc., a boy or girl must be of good character, sound health, between the ages of 8 (plus entering the third year of elementary school) and 17 years, inclusive. Disclosure of all medical and psychological treatments, past and present, is required before a camper arrives at camp. Failure to provide complete a full disclosure of all medical history past and present is grounds for immediate dismissal without refund.
